Essential Job Functions Duties and Responsibilities

Care Plan and Assessment Functions

• Conduct and coordinate the development and completion of the Minimum Data Set (MDS) assessment in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation and completion of CAAs and CATs.
• Manages the reimbursement process and works collaboratively with the Business office and Therapy Department
• Assist with maintaining and updating written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
• Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.
• Participate in facility surveys (inspections) made by authorized government agencies.
• Ensures facility is in compliance with the RAI process
• Work with the Interdisciplinary Care Plan Team in completing a comprehensive resident assessment and care plan for each resident.
• Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
• Assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members. Ensure that care provided is in accordance with the resident's wishes and interests.
• Develop and participate in the planning, conducting, and scheduling of timely in service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility.
• Assist the In‑service Director/Educator in developing any training activities needed concerning resident assessment/care plan skills (including, but not limited to initial or refresher courses relative to techniques for interviewing residents, rehabilitation principles, commonly used psychotropic drugs, care plan functions, etc.).
• Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Ensure that appropriate health professionals are involved in the assessment.  Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident's plan of care.
• Monitor the care plan so that it includes measurable objectives and timetables to meet the resident's medical, nursing, and mental and psychosocial needs as identified in the resident's assessment.
• Develop the schedule of activities required for the resident assessment and plan of care.
• Assist the Director of Nursing and relevant supervisors of other departments in ensuring that all personnel involved in providing care to the resident are aware of the resident's care plan and that nursing personnel refer to the resident's care plan prior to administering daily care to the resident.
• Monitor quarterly and annual resident assessments and care plan reviews So that they are completed timely. Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status.
• Monitor that medical and nursing care is administered in accordance with the resident's preferences and health care provider orders.

 Personnel Functions

• Perform employee performance evaluation.
• Determine MDS staffing requirement, and making recommendations to the DON concerning employee dismissals, transfers, etc.
• Review and evaluate your team’s work process and work to improve.
• Develop work assignments and/or assist in completing and performing such assignments.
• Submit complaints and grievances DON.

The primary purpose of this position is to assist in planning, organizing, implementing, evaluating, and directing  the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Social Services and/or Executive Director, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Social Services Functions

• Conduct an initial social service assessment including MMSE, social history, intake for MDS, and admit note.
• Review nursing charts and complete MDS within facility timelines.
• Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
• Ensure that all social services personnel are aware of the care plan and that care plans are used in providing daily social services to the resident. Review nurses' notes to determine if the care plan is being followed. Report problem areas to the Director of Nursing.
• Evaluate social and family information and assist in determining plans for social treatment. Interview resident/families to obtain social history. Involve the resident/family in planning social service programs when possible.
• Assist in the development, administering, and coordinating of department policies and procedures. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to Executive Director.
• Participate in discharge planning, development and implementation of social care plans and resident assessments
• Provide information to resident/families as to medicare/medicaid, and other financial assistance programs available to the resident.
• Coordinate social service activities with other departments as necessary.
• Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
• Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members.
• Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
• Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the service.
• Ensure that social service work areas are maintained in a clean and sanitary manner.
• Recommend to the Director the equipment and supply needs of the department. Place orders for equipment and supplies as necessary or as may be required.
• Assist in arranging transportation to other facilities when necessary.

The primary purpose of this position is to provide each of your assigned residents with routine daily nursing care and services accordance with the resident’s assessment and care plan, and as may be directed by your supervisors.

Essential Job Functions, Duties and Responsibilities

• Perform simple procedures as an extension of therapy services under the direction and supervision of the nurse, e.g. range of motion exercises, assistance in ambulation or exercises.
• Performs personal care activities including bathing, grooming, dressing, toileting, skin care/nail care, oral hygiene, shaving, dressing, and others as directed/requested.
• Performs household services essential to health care at home including meal preparation/feeding, laundry and ironing, changing/laundering bed linens, light housekeeping and other services as offered.
• Initiates and participates in leisure activities provided for residents.  Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents.
• Assists in the administration of medications under the direction and supervision of the nurse (per state regulations and facility policy).
• Observes and documents the health and emotional condition of each resident, promptly reporting all changes to the nurse.
• Responds promptly and courteously to resident requests for assistance and emergency pull cords.
• Promotes personal safety and a safe environment for residents by observing infection control practices, following facility guidelines, and reporting unsafe situation to the nurse.
• Assist residents in preparing and transporting for appointments and social programs.
• Assist with moving and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Assist residents to walk with or without self help devices as instructed.
• Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
• Provide special resident services offered by the facility.
• Attend and participate in in-services as required by federal, state and company requirements.
• Maintain a work environment that is clean and presents a professional image.
• Follow established safety precautions in the performance of all duties.

• Pour coffee, tea and juices while assisting residents with opening containers.
• Prepare and deliver snacks, etc., as instructed.
• Bus tables during and after the meal.
• Clean work area according to cleaning schedules and assignments.
• Assist in maintaining food storage areas in a clean and properly arranged manner. Stock food and supplies for next meal.
• Provide support services to other dining rooms and special programs as needed.
• Assist chef with preparing and/or dishing up food, garnishing plates, preparing and delivering room service trays, etc.
• Assist with the set up and refill of catering and/or buffet functions.
• Attend and participate in service training programs to meet state and federal requirements.

The primary purpose of this position is to ensure a quality dining experience by greeting by seating residents and guests, overseeing dining room staff and maintaining a pleasant and clean dining environment and in accordance with current applicable federal, state, local regulations and with our established policies and procedures.

• Assists cooks in daily operation of the Dining Room.
• Provides supervision for special functions.
• Coordinates staffing schedules to ensure staffing requirements are met on a daily basis.
• Assigns wait staff to designated sections and all staff to appropriate side work.
• Ensures smooth and timely opening of the dining room, including the facilitation of the pre-meal meeting.
• Actively assists wait staff and bussers , which includes filling shifts and working on the floor. 
• Greets and seats residents and guests.
• Inspects side work and closes dining room.
• Employs appropriate loss prevention procedures.
• Ensures appropriate check control and resident charge procedures.
• Ensures consistently high quality customer service in the Dining Room.
• In consultation with the Executive Chef, hires, trains, disciplines and terminates departmental employees.
• Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment and the achievement of safe working practices.

The primary purpose of the Administrator in Training position is to develop the knowledge, skills and ability to successfully become an Executive Director at one of the communities professionally managed by The Goodman Group when a position becomes vacant.  Depending on background and experience, this will be a 12-18 month training program.

Assignment of management and supervisory functions on a routine basis after the core learning objectives are mastered.

Ability to travel to any of the communities within The Goodman Group portfolio and stay at a designated location for an extended amount of time.

Work with the designated Preceptor to complete tasks for each learning objective required to master each section.

Work with designated Executive Director or Department Manager at a community within The Goodman Group portfolio to gain an understanding of learning objectives.

Submit written commentary to the applicable Regional Director of Operations upon completion of each learning objective rotation.

Once learning objectives have been mastered, focus will shift to the following areas but are not limited to:

Help out as Assistant ED at a building that is undergoing construction or remodeling.

Work with each of the RDO’s by assisting them for up to a month at a time to help a building in their region achieve better success or whatever the RDO determines as a focus area.

Work through a national “project” as determined by the VP of Sr. Living & Health Care and/or RDO.

Help to identify various industry opportunities for awards, grants, etc.  and work with the sales team to ensure we are recognized in the appropriate publications and media.

Guidelines

• The Administrator in Training should apply the theories, concepts, principles and techniques learned through formal academic preparation for the position to practical situations in the community.
• In collaboration with the preceptor, the Administrator in Training should become involved in decision-making activities of increasing difficulty and should have the opportunity to implement decisions which are made.
• Through observation and where possible, the Administrator in Training should increase his/her knowledge and appreciation of the clinical aspects of delivering quality long-term care services.  This would include involvement with nursing service, rehabilitative service and social service and is particularly critical in effecting the integrative role of the administrator.
• The Administrator in Training should develop a familiarity with the resident population in the community and with the unique problems associated with the delivery of multiple services to the senior population.
• The Administrator in Training should become familiar with all departments and/or services in the community (dining services, housekeeping, maintenance, etc.) and be well apprised not only of the individual functions but also of their interface with one another.
• The Administrator in Training must complete all projects and assignments relevant to the Administrator in Training experience made by the preceptor.
• Administrator in Training must communicate openly and objectively with the preceptor at all times, remembering the preceptor is the teacher and ultimately responsible for the community

The primary purpose of this position is to perform the daily activities of the Housekeeping Department in accordance with current federal, state, and local standards, guidelines and regulations governing our facility, and as may be directed by the Executive Director, and/or the Director of Housekeeping, to assure that our facility is maintained in a clean, safe, and comfortable manner.

Essential Job Functions Duties and Responsibilities

Janitorial Services

• Perform daily janitorial functions as assigned.
• Clean and sanitize all equipment, carts, and wheelchairs daily.
• Clean floors to include sweeping, damp/wet mopping, disinfecting, etc.
• Clean walls by washing, wiping, dusting, spot cleaning, disinfecting, deodorizing, etc.
• Remove dirt, dust, etc. from surfaces in soiled linen room using proper cleaning/disinfecting solution.
• Ensure that work assignment areas are clean and that equipment, supplies, etc. are properly stored at all times
• Discard infectious wastes into appropriate containers.
• Sort soiled laundry, linen, garments, etc. and separate those items that require special stain removal/treatment.
• Coordinate daily housekeeping services with nursing services when performing routine cleaning assignments in resident living and/or recreational areas.

Equipment and Supply Functions

• Ensure that an adequate supply of housekeeping supplies to perform daily tasks is maintained in utility/janitorial closets.
• Report burned out light bulbs, exit lights, overhead lights, fluorescent lights, room call lights, etc., to your supervisor as soon as practical.
• Assist others in lifting heavy equipment, supplies, etc., as directed or requested.

The primary purpose of this position is to provide assistance in all dining service functions as directed/instructed and in accordance with established dietary policies and procedures

Dining Service Functions

Transportation

• Provide safe transportation to and from activities/appointments for residents as scheduled.
• Pickup and deliver items as directed to meet the needs of the community.
• Assist each resident on and off the vehicle using proper safety measures and devices.
• Responsible for daily inspection and maintenance of the community vehicle (gas, oil, coolant, air pressure, lights, etc.) and cleanliness of the interior and exterior.
• Identify transportation issues/concerns to the Life Enrichment Director or Executive Director.
• Assist others in lifting heavy equipment, supplies, etc. using proper safety measures.
• Assist activities in other departments when not scheduled to drive.
• Attend and participate in service training programs to meet state and federal requirements.

The primary purpose of this position is to greet and assist visitors, residents, callers and provide exemplary customer service at all times.  As directed by your supervisor, perform administrative support functions efficiently and in accordance with established procedures.

Administrative Functions

• Greet visitors, residents, and vendors in a courteous and professional manner.  Identify visitors and direct to appropriate location.  Process and record visitor information as defined by the community.
• Answer the telephone, determine nature of call, and direct caller to appropriate individual or department.
• Respond to requests from within the community; identify and locate appropriate personnel.
• Maintain resident information by name and room number, eligibility to receive calls, emergency phone members of on‑call personnel, department extensions, key personnel, etc.
• Exercise discretion in releasing information so that confidentiality and privacy are maintained.  Information must be stored in a safe and secure manner at all times.
• Identify security concerns and notify supervisor or proper authorities.  Effectively communicate facility policies and procedures to visitors, staff and vendors.
• Assist with administrative duties which includes, typing, copying, filing, posting accounts, sorting mail, ordering supplies and repairs, mailing out doctor’s orders.
• Maintain a work environment that is clean and presents a professional image.
• As appropriate, document and resolve concerns or relay to the appropriate staff.
• Ethically and responsibly conduct small monetary transactions for ticket sales, etc
• Attend and participate in service training programs to meet state and federal requirements.
